Individual and Family Plan Information

Individual and Family PPO Medical Plans – With a PPO (preferred provider) health plan, you’ll pay a lower share of your medical expenses when you use doctors or hospitals that participate in our PPO provider network.

Individual Short-Term PPO Plans – Between jobs? Just graduated? Our Short-Term PPO plans provide coverage from 30 to 180 days, allow you to begin or end coverage any day of the month, and offer deductibles from $250 to $2,000. The policy is non-renewable and designed to meet your health plan needs while you are between other coverage.

Individual and Family HMO Medical Plans – HMO stands for Health Maintenance Organization. With this kind of health care plan, you’ll choose a Primary Care Physician (PCP) from our HMO network. Your PCP will probably be the doctor you see the most – for routine visits and care. They will also coordinate any other health care services you may need. And if you need to see a specialist, your PCP will need to make a referral.
